Each year every congregation is required by its constitution to compile an annual report of the previous year and submit it to the churchwide and synod offices. This is information from your financial records and parish register. The forms can be accessed from the link below and may be filed electronically or by mail. The deadline for the report is February 15 of every year. Thank you for submitting your report in a timely manner.
